Upon scheduling, you will receive an email confirmation with a link to fill out an intake form. This is a requirement for all new clients/patients and should be done online prior to your first visit. Please note: Insurance coverage must be verified prior to your initial appointment at least 72 hours in advance. Referrals or prescriptions may be required.
