New Patient Info

Easy online booking for any service available below.

Upon scheduling an appointment, you’ll receive an email confirmation with a link to open an intake form. All new clients/patients will need to fill out the form, which can be done online before your visit for your convenience.
Please note: Insurance benefits must be verified before your appointment time so it’s best to inquire with your info a few days prior to your visit.  Please be aware,       a referral/prescription may be required before the visit.

Insurance patients:
Currently accepting new patients with massage benefits under the following companies:
Regence Blue Cross Blue Shield
BlueCross BlueShield Federal Employee Program (FEP)
Premera Blue Cross
Regence BCBS of Oregon
Providence Health Plan


Car Accident claims:
Please bring PIP claim number, adjustor name and number, and any referrals or prescription for massage (highly recommended).

For massage benefit inquiries, send an email to with the following information:
A picture of your insurance card, your birth date, and full name,
and we will get back to your within 24-48 hours with your eligibility.
